Pre-Participation Requirements

Clearance for Intercollegiate Athletics Participation

Preparticipation information for incoming student-athletes (2025-2026 academic year) will be released in June of 2025.  At that time, you will receive communication via email from Head Athletic Trainer, Kara Campbell, with instructions on how to complete all requirements.  You DO NOT need to have a sports physical done prior to arrival as those will be completed on campus before to the start of your sports season.  If you have any questions, please email karacampbell@depauw.edu.      

Student-athletes must participate in Pre-Participation Examinations annually.

PLEASE NOTE:  IN ADDITION to completing the Sports Medicine information and forms below, ALL first-year student-athletes must complete the immunization forms for DePauw Health.  You can find the information on the DePauw Health forms here.

The following is for First Year and Transfer student-athletes new to DePauw University Athletics for the 2025-26 Academic Year. Please READ below and FOLLOW the directions to be cleared for participation. The information required below must be completed by July 1st, 2025.

Log on to the DePauw University Sports Medicine electronic medical record website
  • Go to: depauw2.atsusers.com (**do not use www.)
  • Athlete ID: Type 'New'
  • Password: Type 'New'
  • Database: atsdepauw (should already be filled in)
Fill in required "General" tab information (light yellow colored items are required to be filled out)
  • Select team: identify all teams if you play multiple sports
  • DePauw Email Address
  • Home Address, City, State, and Zip Code
  • Year in School: select Freshman; if a transfer then please select appropriate year in school
  • Athlete ID: please use your FULL school issued student ID 
  • Password: can be anything you like, but must be at least 8 characters in length 
  • Fill out any medical alerts, allergies, or medications. If you do not have any, please write NONE in those boxes.
  • Click the save button. You may have to do this twice. A dialog box will appear indicating your information has been saved
Fill in required "Sickle Cell" tab information (light yellow colored items are required to be filled out)

The NCAA now requires that all incoming student-athletes must be tested for sickle cell trait and provide proof of results.  To complete this requirement, all first-year and transfer student-athletes are required to upload results of sickle cell solubility blood test into your ATS account.    

  • Newborn Screening Test Results: Most states required newborn SCT testing beginning in 1990. Contact your hospital of birth/pediatrician's office and request a copy of your sickle cell trait status from your birth. Upload a picture of the results.
    • If you were born in Indiana:  Click here for information
      • Please note, DePauw's Team Physician is not responsible for faxing requests for SCT results.  This must be done by your personal physician.
    • If you were born elsewhere in the United States, and unable to obtain results through your healthcare provider, you can directly request your results through your state's health department. 
  • Get tested: Upload a picture of a recent blood test indicating your Sickle Cell Trait status in your ATS account. This requires a written request from a physician, a blood draw from a physician's office or lab, and a lab test with results indicated.
Fill in "Insurance" tab information: (light yellow colored items are required to be filled out).  You must have primary health insurance coverage to participate in intercollegiate athletics at DePauw. Please note that Medicaid, Tricare, and medical/healthcare cost-sharing plans do not meet the university's minimum requirement for athletics participation. The university does have an individual sports plan policy available for purchase - $840 - and it can be purchased at the EIIA website. The 2025-2026 enrollment period will open soon. Please contact your staff Athletic Trainer if you do not have primary health insurance
In addition to submitting primary health insurance in ATS, ALL new and returning student-athletes must complete the University's insurance waiver process. You can find more information here
  • Company (Click "Add new insurance company" button ONLY IF your insurance company is not listed)
  • Phone: Please enter the Customer Service Phone Number
  • Both ID# and Group# (If only one is listed on your insurance card, please enter that in the correct field)
  • At the bottom of the page, upload a new picture of the FRONT of your insurance card in the 'Card Front' tab. Picture needs to be in a landscape orientation and cropped so only the card shows
  • At the bottom of the page, upload a new picture of the BACK of your insurance card in the 'Back' tab. Picture needs to be in a landscape orientation and cropped so only the card shows
  • Click the Save button
    • Please make sure the pictures are legible
Fill in "Contacts" tab information (light yellow colored items are required to be filled out)
  • Please provide one primary emergency contact in this step
    • Additional emergency contacts may be added once your account is created
Under the "Forms" tab, click the drop down box by 'Form Name' and then click 'New' for each of the 2 required forms

YOU MUST COMPLETE EACH OF THE FOLLOWING "FORMS"

  • Pre-participation - First Years or Transfers
  • General Release and Waiver

Upon completion of each of the forms, you must electronically sign it by agreeing in the check box,  typing your name in the "signed by" box and then click the sign button. Parent e-signatures are not required for student-athletes age 18 and over. Lastly, click the save button at the bottom of the page (Click it ONE TIME ONLY).  Please save fairly quickly as it can 'time out' if too much time has passed once complete.  If necessary, you may always go back in and update the form after it has been saved.

Under the "eFiles" tab, please read below and complete all that apply to you. 

ADD/ADHD: If you have been diagnosed with ADD/ADHD, and take prescribed medication,  you will need to download the "Initial Documentation" file and have it completed by your prescribing physician. The initial documentation form is linked here. Once completed, please upload the paperwork back in to Athletic Trainer System under the "eFiles" tab. 

Asthma: If you have Asthma and have been prescribed an inhaler, you will need to obtain a copy of the script from the prescribing physician.  You will then need to upload that copy in to Athletic Trainer System under the "eFiles" tab.
