Clearance for Intercollegiate Athletics Participation

2025-26 Returning Student-Athletes

DePauw Student-athletes participate in Pre-Participation Examinations (PPEs) annually. The information below is required for the 2025-26 academic year. 

  • Log on to the DePauw University Sports Medicine electronic medical record website
    • Click here: depauw2.atsusers.com (the website will open in a new tab)
    • Athlete ID: Student ID Number
    • Password: Type in your password (click the 'Forgot Your Password' link if you cannot remember yours)
      • If you had to change your password because you forgot it, then you need to refresh your browser before attempting to log in again

Once logged in, click on the Athlete Information Icon and continue to follow the instructions below:

  • General tab
    • Review and update, if necessary, the information in the highlighted boxes.  If you update any information, click 'save' at the bottom of the page

  • Insurance tab - Please note that out of state Medicaid, Tricare and medical/healthcare cost-sharing plans do not meet the university's minimum requirement. In addition to submitting primary health insurance in ATS, ALL new and returning student-athletes must complete the University's insurance waiver process. You can find more information here.
    • Review the information
      • if no changes are necessary, you may skip to the 'contacts tab' section
    • If your health insurance has expired or changed in any way please follow the instructions below:
      • click on 'company' to highlight the insurance name
      • click 'edit'
      • edit all information in the highlighted boxes
        • you must have an accurate policy end date (if you do not know the end date, you should call the number on the back of the insurance card or speak with the policy holder)
      • upload a picture of the front and then the back of your insurance card at the bottom of the page
        • click the 'upload' tab 
        • picture needs to be in landscape orientation and cropped so only the card shows
      • Save

  • Contacts tab - these are emergency contacts
    • confirm name, phone numbers, and email addresses.  If changes are made, click 'save' at the bottom of the page.
      • You may add, edit and delete entries by using the blue buttons
    • If no changes are necessary, click 'verify emergency contact information.

  • Forms tab - You need to complete the Pre-Participation - Returner form as well as the General Release and Waiver form
    • A NEW returner PPE form must be completed for the 2025-26 academic year; follow the directions below (DO NOT update the current form that is already completed and saved in the system).
    • Click the form name drop down box and select Pre-Participation - Returner, then click new
    • Read all directions and answer all questions

    • Electronically sign it by agreeing in the check box, typing your name in the 'signed by' box, click sign button, and  then click the save button
    • Once you have saved the Pre-Participation - Returner form you will need to follow the same steps above and complete the General Release and Waiver Form

  • eFiles tab - complete only if you have been diagnosed with ADHD or Asthma (or other conditions treated by taking an NCAA banned substance)
    • If you have been newly diagnosed with ADD/ADHD, and take prescribed medication, then you will need to download the "Initial Documentation" file and have it completed by your prescribing physician. Once completed, please upload the paperwork back in to Athletic Trainer System under the "eFiles" tab.
    • If you have already submitted Initial Documentation, then you will need to download the "Follow Up Documentation" file and have it completed by your prescribing physician.  Once completed, please upload the paperwork back in to Athletic Trainer System under the "eFiles" tab.
    • If you have Asthma and have been prescribed an inhaler, you will need to obtain a signed note from your prescribing Physician indicating the diagnosis, course of treatment, medication and dosage of the medication.  You will then need to upload that note in to the Athletic Trainer System under the "eFiles" tab.
