DePauw Tigers Volleyball Recruiting Questionnaire
In compliance with the NCAA Board of Governors Policy on Campus Sexual Assault violence, all enrolling student-athletes will be required to disclose if they have been involved in a Title IX proceeding, were disciplined through a Title IX proceeding or have a criminal conviction for sexual, interpersonal or other act of violence. The Director of University Compliance will solicit this information and maintain it in confidence; the athletics department and its staff will not have access to this information.
Please remember that this is not the official admission application to DePauw University. If you are transferring to DePauw University from another four-year institution, please remember that you may need "permission to contact" us. If you are transferring from another NCAA Division III institution, you can complete the "Permission to Self Release". Individuals transferring from all other institutions should seek the guidance of their athletics director or director of compliance.
